Please Note: As part of the complaint handling process, the Bureau may send a copy of this form to the individual or company against whom your complaint is filed. Failure to supply your complete and accurate contact information may result in delayed processing of your complaint.

INSTRUCTIONS:

  1. Please complete a separate complaint form for each call.
    Items marked with an asterisk (*)€ indicate information we must have to investigate your complaint.
  2. Please submit your complaint within two weeks of receiving the call.

Registration on the Do Not Call list should reduce the number of unwanted telemarketing calls you receive. Should you continue to receive telemarketing calls not exempt under the law, please report them to our office.
